Job Summary
We are seeking a dedicated Nurse Practitioner or Physician Assistant (PRN) to conduct in-home health assessments. This role is vital in providing comprehensive patient evaluations, health education, and coordinating care for individuals either in their residences or in Skilled Nursing Facilities.
Responsibilities
- Perform thorough health assessments of patients in their homes or other care settings.
- Deliver health education to patients and their families to promote wellness and understanding of medical conditions.
- Coordinate with physicians for referrals and additional medical care as necessary.
- Engage in case management to ensure continuity of care for patients.
- Participate in virtual hiring events to connect with the healthcare community.
Qualifications
- Active Nurse Practitioner or Physician Assistant license in the state of New York.
- Experience in conducting health assessments and case management preferred.
- Strong communication skills and the ability to educate patients effectively.
- Flexibility to work on a PRN basis and adapt to varying patient needs.
- Commitment to improving patient health outcomes through dedicated care.
