Job Summary
Matrix Medical Network is seeking a dedicated Nurse Practitioner or Physician Assistant for PRN positions focused on conducting in-home health assessments in Pittsfield, Maine. This role is integral to our commitment to enhancing patient care and ensuring that health evaluations are effectively performed in various settings, including patients’ homes and Skilled Nursing Facilities.
Responsibilities
- Conduct in-home health assessments to evaluate patients’ medical needs.
- Provide treatment options and health education to patients and their families.
- Facilitate physician referrals and coordinate case management referrals as necessary.
- Ensure thorough documentation and communication regarding patient care and recommendations.
- Collaborate with healthcare teams to optimize patient outcomes.
Qualifications
- Current Nurse Practitioner or Physician Assistant certification.
- Valid state licensure to practice in Maine.
- Experience in performing health assessments and providing direct patient care.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
