Job Summary
The Nurse Practitioner or Physician Assistant (PRN) role involves conducting in-home health assessments within the Montross, Virginia area. The position is embedded within Matrix Medical Network, an organization that has pioneered national in-home healthcare services. This includes comprehensive health evaluations, diagnosis and treatment, along with health education and coordination of care through physician referrals and case management.
Responsibilities
- Conduct in-home health assessments for patients in various settings including their residences and Skilled Nursing Facilities.
- Deliver diagnosis and treatment plans tailored to individual health needs.
- Provide health education to patients and their families.
- Facilitate referrals to physicians and case management services as required.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Qualifications
- Possession of a valid Nurse Practitioner or Physician Assistant license.
- Experience in providing healthcare services in home or community settings is preferred.
- Strong interpersonal skills for effective communication with patients, families, and healthcare teams.
