Job Summary
The Nurse Practitioner or Physician Assistant role focuses on conducting in-home health assessments for patients in Liberty, New York, as part of a wider initiative at Matrix Medical Network. This position involves interacting with patients directly in their residences or Skilled Nursing Facilities, providing essential healthcare services aimed at improving patient outcomes.
Responsibilities
- Perform comprehensive health assessments in patients’ homes or Skilled Nursing Facilities.
- Provide health education to patients and their families to promote wellness.
- Facilitate physician referrals and manage case referrals as necessary.
Qualifications
- Must hold a valid Nurse Practitioner (NP) or Physician Assistant (PA) license.
