Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) involves conducting in-home health assessments in the Fort Edward, New York area. This role is part of the Matrix Medical Network, providing critical health services directly to patients in their homes or in Skilled Nursing Facilities.
Responsibilities
- Conduct comprehensive health assessments in patients’ homes and Skilled Nursing Facilities.
- Provide treatment, health education, and support for ongoing healthcare needs.
- Facilitate physician and case management referrals to ensure continuity of care.
Qualifications
- A valid Nurse Practitioner or Physician Assistant license.
- Experience in conducting health assessments and patient management.
- Strong communication and interpersonal skills to engage effectively with patients and healthcare teams.
