Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) focuses on conducting in-home health assessments. Located in Chase City, Virginia, this role requires onsite presence to deliver comprehensive health services within the community. Responsibilities
- Conduct health assessments in patients’ homes or at skilled nursing facilities.
- Provide treatment and health education to patients.
- Facilitate physician referrals and case management services.
Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant.
- Experience in delivering in-home health services is preferred.
