Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) involves conducting in-home health assessments in Saluda, Virginia. This role is essential for providing patient-centered care and ensuring appropriate health interventions are made in a familiar environment.
Responsibilities
- Perform in-home health assessments for patients.
- Deliver essential health education and treatment.
- Manage physician referrals and case management referrals.
- Provide care in patients’ homes or in Skilled Nursing Facilities.
Qualifications
- Licensure as a Nurse Practitioner or Physician Assistant.
- Experience in conducting health assessments.
