Job Summary
The role involves performing thorough in-home health evaluations for elderly and high-risk populations to detect chronic health issues and enhance patient care results. The healthcare providers will work in conjunction with primary care doctors, caregivers, and other medical staff to facilitate proper follow-up and maintain accurate documentation.
Responsibilities
- Conduct in-home health assessments to identify chronic conditions in older adults and at-risk individuals.
- Collaborate with primary care physicians and medical staff for effective patient care management.
- Ensure thorough documentation and follow-up procedures to improve health outcomes.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in a related specialty area.
- Current certifications in BLS, ACLS, or CPR.
