Job Summary
The position involves carrying out thorough in-home health evaluations for elderly individuals and those at risk, focusing on their medical backgrounds to detect chronic illnesses. Providers will work in partnership with primary care doctors and caregivers to facilitate proper follow-up and enhance care management.
Responsibilities
- Perform comprehensive health assessments for seniors and at-risk populations.
- Review and analyze patients’ medical histories to identify chronic conditions.
- Collaborate with primary care physicians to ensure holistic patient care.
- Coordinate with caregivers to support effective follow-up and management of care.
Qualifications
- Master’s degree in a relevant field.
- Valid Nurse Practitioner licensure with prescriptive authority.
- Board certification in an appropriate specialty area.
- Current certifications in BLS, ACLS, or CPR.
