Job Summary
This role involves conducting thorough in-home health evaluations for adult and elderly patients, focusing on reviewing their medical histories, recognizing chronic ailments, and addressing any care discrepancies. Responsibilities
- Perform comprehensive in-home health assessments for adult and geriatric members.
- Review medical histories to identify chronic conditions.
- Identify and address care gaps.
- Collaborate effectively with primary care providers, caregivers, and social service professionals.
- Ensure accurate clinical documentation is maintained.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in an applicable specialty.
- Possession of a valid driver’s license and automobile insurance.
- Strong computer skills necessary for documentation and communication.
