Job Summary
The position involves conducting thorough in-home health evaluations for senior adults and individuals at risk. The primary focus is to analyze medical histories, assess medication compliance, and identify social risks, ultimately enhancing healthcare outcomes for the patients served.
Responsibilities
- Perform in-depth health assessments in clients’ homes for older adults and at-risk individuals.
- Evaluate and document patients’ medical histories and medication adherence.
- Identify and address social risk factors affecting health.
- Collaborate effectively with primary care providers and caregivers to enhance patient care.
- Work actively on closing health care service gaps for better health outcomes.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with the authority to prescribe medication.
- Board certification in a pertinent specialty area.
- Valid BLS, ACLS, or CPR certification.
