Job Summary
The role involves performing detailed in-home health evaluations for elderly clients and individuals at risk, aiming to recognize chronic health issues and enhance overall care results. The professional will work in partnership with primary care doctors and caregivers to ensure effective management of patient health, adherence to medication protocols, and coordination of follow-up services.
Responsibilities
- Conduct thorough in-home health assessments to determine chronic health conditions in older adults and at-risk patients.
- Collaborate effectively with primary care physicians and caregivers for managing patient care.
- Ensure and encourage adherence to prescribed medication regimens.
- Coordinate and facilitate necessary follow-up care for patients.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in an applicable specialty.
- Current certifications in BLS, ACLS, or CPR.
