Job Summary
The Provider’s primary role is to perform detailed health evaluations within the home settings of elderly adults and individuals at risk, focusing on identifying chronic illnesses and addressing any gaps in their care. This position requires effective collaboration with various healthcare professionals, including primary care physicians, caregivers, and social workers, ensuring that all care provided is well-coordinated and documented accurately.
Responsibilities
- Conduct thorough in-home health assessments for elderly and at-risk patients.
- Identify chronic health conditions and work to bridge care gaps.
- Collaborate closely with primary care physicians, caregivers, and social workers.
- Ensure accurate documentation and coordination of care for patients.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner license with prescriptive authority.
- Board certification in an appropriate specialty.
- Up-to-date certifications in BLS, ACLS, or CPR.
