Job Summary
The role involves performing thorough in-home health evaluations for adult and elderly patients to assess their medical backgrounds, medication compliance, and social risk factors. Providers work in partnership with primary care doctors and other healthcare professionals to enhance the quality of care and address health disparities.
Responsibilities
- Conduct detailed health assessments in patients’ homes, focusing on adults and the elderly.
- Evaluate medical histories, medication adherence, and identify potential social risks.
- Collaborate with primary care physicians and healthcare teams to improve patient care.
- Facilitate the closing of health-related gaps in patient care.
Qualifications
- A Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with the authority to prescribe medications.
- Board certification in a pertinent specialty, such as Family or Adult-Gerontology.
- Certification in BLS, ACLS, or CPR.
