Job Summary
The role of the Providers involves executing thorough health assessments for seniors and individuals at risk within their homes. The aim is to identify chronic illnesses and evaluate their medical histories, ensuring holistic care.
Responsibilities
- Conduct detailed in-home health evaluations for older adults and at-risk populations.
- Identify and document chronic conditions while reviewing patients’ medical histories.
- Work in collaboration with primary care physicians, caregivers, and other healthcare professionals to maintain quality care.
- Facilitate appropriate follow-up care based on assessment findings.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with the authority to prescribe medication.
- Board certification in an applicable specialty.
- Current certifications in BLS, ACLS, or CPR.
