Job Summary
This role involves carrying out thorough in-home health evaluations for senior citizens and individuals at risk, focusing on their medical background and recognizing chronic health issues. The position emphasizes teamwork with primary care doctors and caregivers to enhance health outcomes and address care deficiencies.
Responsibilities
- Perform detailed in-home health assessments for elderly and at-risk populations.
- Review patients’ medical histories to identify chronic conditions.
- Collaborate with primary care providers and caregivers to improve patient health outcomes.
- Identify and help close gaps in patient care.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner license with prescriptive authority.
- Board certification in a pertinent specialty.
- Current certifications in BLS, ACLS, or CPR.
