Job Summary
The role involves performing thorough in-home health evaluations for adult and elderly patients. The primary focus is on identifying existing chronic conditions and potential health risks to enhance patient care.
Responsibilities
- Execute detailed health assessments for adult and geriatric individuals in their homes.
- Identify chronic health conditions and risks to inform care plans.
- Work collaboratively with primary care physicians and other healthcare providers to ensure seamless follow-up.
- Engage with caregivers and medical teams to improve overall patient management and care strategies.
Qualifications
- A Master’s degree in nursing or a related field.
- Current licensure as a Nurse Practitioner with authority to prescribe medications.
- Board certification in a relevant nursing specialty.
- Current certifications in Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), or Cardiopulmonary Resuscitation (CPR).
