Job Summary
The role involves performing detailed health evaluations in the homes of elderly individuals and those at risk to uncover chronic health issues and bridge care gaps. Providers work closely with primary care doctors, family members, and social workers to facilitate well-coordinated care and maintain accurate documentation.
Responsibilities
- Conduct thorough in-home health assessments for older adults and at-risk patients.
- Identify chronic health conditions and address care gaps.
- Collaborate with primary care physicians and other healthcare professionals.
- Ensure clear and proper documentation of health assessments.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with board certification in a pertinent specialty.
- Valid driver’s license and automobile insurance.
- Strong proficiency in computer skills.
