Job Summary
The role involves conducting thorough in-home health evaluations for adult and elderly patients to detect chronic illnesses and potential health hazards. Providers are required to work closely with primary care physicians, caregivers, and healthcare professionals to ensure integrated care and accurate record-keeping.
Responsibilities
- Perform in-depth health assessments in patients’ homes.
- Identify and evaluate chronic conditions and health risks amongst adult and geriatric populations.
- Collaborate effectively with primary care doctors and caregivers for optimal patient care.
- Maintain precise documentation and communication with the healthcare team.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with the ability to prescribe medications.
- Board certification in a pertinent specialty.
- Up-to-date BLS, ACLS, or CPR certification.
