Job Summary
The role involves conducting thorough in-home health assessments for both adult and elderly patients, focusing on recognizing chronic conditions and potential gaps in care. Responsibilities
- Perform detailed health assessments in the home environment for adult and geriatric members.
- Identify chronic health conditions and any existing gaps in patient care.
- Collaborate with healthcare teams including primary care doctors, case managers, and caregivers for integrated patient care.
- Ensure accurate medical documentation is maintained throughout the care process.
Qualifications
- A Master’s degree in nursing or a related field.
- Current Nurse Practitioner licensure with authority to prescribe medications.
- Board certification in a relevant specialty is mandatory.
- Must possess a valid driver’s license and appropriate automobile insurance.
