Job Summary
This role involves performing thorough in-home health evaluations for adult and elderly patients, focusing on their medical history, adherence to medication, and identifying potential social risks. The position requires teamwork with primary care doctors and healthcare staff to deliver education, referrals, and comprehensive clinical documentation.
Responsibilities
- Conduct in-depth health assessments for adult and geriatric patients in their homes.
- Evaluate patients’ medical histories and medication adherence.
- Identify and assess social risks affecting patient health.
- Collaborate closely with primary care physicians and other healthcare providers.
- Provide necessary education and referrals to patients.
- Maintain clear and accurate clinical documentation of patient assessments.
Qualifications
- Master’s degree in Nursing or a related field.
- Current Nurse Practitioner licensure with authority to prescribe medication.
- Board certification in a pertinent specialty, such as Family Medicine or Adult Gerontology.
- Certification in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Cardiopulmonary Resuscitation (CPR).
