The Nurse Practitioner will perform comprehensive health assessments, diagnose acute and chronic conditions, and develop individualized care plans for residents in skilled nursing facilities. They will also collaborate with an interdisciplinary team to manage patient care, reduce hospitalizations, and ensure high-quality clinical outcomes. Candidates must hold a Master’s degree in Nursing from an accredited program and possess an active, unrestricted Advanced Practice Nurse license. At least two years of clinical experience in geriatrics, long-term care, or post-acute settings is preferred.
