**Hospice RN Case Manager**
The Hospice RN Case Manager is responsible for orchestrating and overseeing patient care processes, delivering skilled nursing services while ensuring the effective execution of care plans. This role involves working in conjunction with a multidisciplinary team to promote patient comfort, uphold dignity, and maintain adherence to regulatory standards. Applicants must possess an active Registered Nurse (RN) license and have a minimum of one year of experience in hospice care or a similar healthcare environment. Essential qualifications include robust clinical assessment abilities, excellent communication skills, and strong critical thinking capabilities, in addition to a valid driver’s license and CPR certification.
