Job Summary
The Part-Time Hospice RN Case Manager plays a vital role in delivering high-quality end-of-life care. This position involves collaborating with patients and their families to develop and implement individualized plans of care, ensuring compassionate support and effective management of symptoms.
Responsibilities
- Assessing patients’ health status and identifying care needs.
- Creating and overseeing personalized care plans in conjunction with the healthcare team.
- Providing education and support to patients and families regarding hospice services.
- Coordinating with interdisciplinary teams to facilitate comprehensive care.
- Monitoring and evaluating the effectiveness of care plans and making adjustments as necessary.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in hospice or palliative care is preferred.
- Strong communication and interpersonal skills to interact effectively with patients and families.
- Ability to work collaboratively in a team-oriented environment.
- Compassionate nature and a commitment to patient-centered care.
