Job Summary
The Physical Therapist is responsible for delivering physician-ordered therapy services to patients in their residences, aimed at enhancing physical capabilities and rekindling functional independence. This role encompasses initial evaluations, creation of individualized care strategies, and oversight of clinical staff.
Responsibilities
- Conduct comprehensive assessments of patients’ physical abilities.
- Develop tailored therapy plans based on physician directives.
- Provide therapy services to promote recovery and rehabilitation.
- Supervise and guide clinical team members in therapy execution.
- Ensure thorough documentation and adherence to regulatory standards.
Qualifications
- Degree in physical therapy from an accredited educational institution.
- Current licensure to practice as a physical therapist in the state.
- Minimum of two years’ experience in a home health environment is preferred.
- Familiarity with Medicare OASIS documentation processes.
