Job description
The physician will deliver primary preventive care and manage chronic illnesses for adults aged 65 and above within a value-based care framework. They will collaborate with an integrated care team to oversee a patient population that is predominantly insured through Medicare Advantage. Applicants must hold Board Certification or be Board Eligible in Internal Medicine or Family Medicine. While experience in geriatrics, value-based care, and familiarity with HEDIS metrics is desirable, it is not mandatory.
