Job description
Job Title: Primary Care Provider for Geriatric Patients
Deliver comprehensive preventive and chronic disease management for adults aged 65 and older while adhering to a value-based care framework. Work closely with a multidisciplinary care team comprising Nurse Practitioners, Physician Assistants, social workers, and support personnel to ensure coordinated patient care. Applicants should be Board Certified or Board Eligible in either Internal Medicine or Family Medicine. While experience in geriatrics, value-based care, and HEDIS is advantageous, it is not a mandatory requirement.
