The Social Worker is responsible for evaluating the psychosocial requirements of hospice patients and their families, offering emotional support and counseling services. This role involves working closely with the interdisciplinary team to create comprehensive care plans and assist families in accessing crucial community resources. Applicants should possess either a Master’s or Bachelor’s degree in Social Work and hold a valid LMSW or LBSW license in Texas. Prior experience in hospice or palliative care is highly desirable, and candidates must be willing to travel to different patient sites.
