Job Summary
The Recovery Support Coordinator plays a vital role in overseeing a group of members, delivering recovery-related support services and orchestrating care across both clinical and community environments. This position involves working closely with a diverse team of professionals to enhance treatment participation, address health-associated social needs, and ensure precise record-keeping.
Responsibilities
- Manage a designated panel of members to ensure effective recovery support services.
- Coordinate care among clinical settings and community resources.
- Collaborate with a multidisciplinary team to promote treatment engagement.
- Assist members in addressing their health-related social needs.
- Maintain and update accurate documentation related to member services.
Qualifications
- A high school diploma is required; an associate degree in a relevant field is preferred.
- A minimum of two years’ experience in case management or peer support is desirable.
- Proficiency in utilizing electronic health records is important.
