Job description
**RN Case Manager**
The RN Case Manager will deliver direct hospice nursing care while coordinating patient services in the Atlantic City area. This role involves working closely with the clinical team to create tailored care plans and acting as a supportive advocate for both patients and their families. Applicants must possess a valid, unrestricted Registered Nurse license, along with a minimum of one year of experience in either a medical-surgical or acute care environment. Additionally, candidates should be skilled in using EMR systems and hold a current driver’s license and CPR certification.
