**RN Case Manager**
The RN Case Manager plays a critical role in discharge planning and maintaining continuity of care, serving as a bridge among patients, families, and the healthcare team. This position employs management strategies to guarantee that healthcare services are medically necessary, suitable, and delivered efficiently. Applicants are required to have a valid Registered Nurse license in Virginia and a minimum of one year of experience in case management or a clinical setting. A Bachelor’s or Associate’s degree in Nursing is necessary, with an expectation to obtain a BSN within five years for those currently holding an Associate’s degree.
