Job description
The RN Case Manager serves as the key clinical resource for patients, overseeing care coordination and leading the interdisciplinary team. Key responsibilities entail performing thorough assessments, creating tailored care plans, and offering education to patients and their families. Applicants should possess a valid RN license and have completed their education at an accredited nursing institution. A minimum of one year of experience in hospice, home health, or a similar area is desirable.
