Job Summary
The Rehabilitation Case Manager plays a critical role in coordinating and managing client treatment plans from the moment of admission until discharge. This position acts as the central communication hub for families and healthcare professionals involved in the care process.
Responsibilities
- Oversee and coordinate treatment plans for clients throughout their rehabilitation journey.
- Serve as the key contact for families and healthcare providers during the treatment process.
- Facilitate interdisciplinary team meetings to discuss client progress and treatment adjustments.
- Ensure that all clinical documentation adheres to regulatory requirements and quality assurance standards.
Qualifications
- A bachelor’s degree in healthcare or a related discipline is essential.
- A master’s degree is highly preferred.
- Prior hands-on experience in case management is necessary.
- Willingness to travel up to 30% of the time is required.
