Lead clinical care and coordination for the BreakThru medically managed withdrawal program, including assessments, individualized aftercare planning, patient follow-up, and quality improvement. Navigate qualifying surgical episodes under the CMS TEAM program by coordinating transitions, tracking outcomes, supporting CMS quality reporting, and educating staff on program requirements. An ADN and an active Kansas RN license (or eligibility for licensure by endorsement) are required, along with BLS certification and at least two years of clinical nursing experience. A BSN and ACLS are preferred; experience in medical/surgical care, behavioral health, substance use treatment, or care coordination is strongly preferred, and CARN or a similar certification is a plus.
