Oversee a portfolio of Family Care Program participants by performing thorough clinical evaluations and creating individualized care strategies. Ensure continuous care coordination, offer member education, and facilitate quality assurance oversight while working alongside a multidisciplinary team. An Associate’s Degree in Nursing and a valid Registered Nurse license in Wisconsin is required. Applicants should possess a minimum of 3 years of recent experience with older adults or individuals with disabilities.
