Job Summary
The RN Case Manager plays a vital role in delivering extensive case management for individuals with developmental and intellectual disabilities. This position is focused on ensuring that clients’ healthcare requirements are addressed through effective advocacy and coordination of services.
Responsibilities
- Coordinate case management services to meet the healthcare needs of clients with developmental and intellectual disabilities.
- Advocate for clients to ensure they receive appropriate care and services.
- Supervise and support LPNs and Direct Support Professionals in their roles.
- Maintain accurate documentation related to client care and services.
- Ensure compliance with CMS regulations in all aspects of care management.
Qualifications
- Candidates must possess a current Registered Nurse license in Tennessee.
- A valid driver’s license is required for this position.
- Must be able to obtain BLS and Professional Crisis Management certifications within the first 60 days of employment.
