Job Summary
The Clinical Liaison plays a crucial role in overseeing the intake and referral processes within the healthcare framework. This position involves evaluating patient eligibility and liaising with healthcare providers to facilitate admissions. The Clinical Liaison is also responsible for fostering and developing strong connections with referral sources to enhance community understanding of hospice services.
Responsibilities
- Assess patient eligibility for hospice services.
- Coordinate admission processes in collaboration with healthcare professionals.
- Build and maintain relationships with referral sources to promote hospice services.
- Increase community awareness regarding hospice offerings.
Qualifications
- Current Registered Nurse (RN) license is required.
- Three to five years of experience in hospice care is needed.
- A Bachelor of Science in Nursing (BSN) is preferred.
- Experience in discharge planning or case management is advantageous.
