Job Summary
The Clinical Liaison plays a crucial role in overseeing the intake and referral procedures, ensuring patient eligibility assessments and coordination of admissions in collaboration with healthcare providers. This position also focuses on establishing and nurturing robust connections with referral sources through regular visits to various hospitals and facilities.
Responsibilities
- Manage and streamline the intake process, assessing patient eligibility for hospice services.
- Coordinate admissions in partnership with healthcare professionals to facilitate smooth transitions.
- Develop and sustain strong relationships with referral sources through daily outreach and visits.
- Engage with hospitals and other healthcare facilities to promote the hospice services.
Qualifications
- Must possess a valid Registered Nurse license.
- Have a minimum of three to five years of experience in hospice care.
- Need to hold a valid driver’s license and automobile insurance.
- CPR certification is required.
