The Home Care Case Manager is responsible for coordinating and providing home care services as ordered by a physician. They must implement care plans, maintain accurate documentation, and communicate patient status to the healthcare team. Candidates must hold a current Ohio Registered Nurse license and have 1-3 years of clinical experience, preferably in an acute care setting. Proficiency in IV skills and the ability to perform physical tasks like lifting up to 40 lbs are required.
