**Job Title: Care Manager**
The Care Manager is responsible for overseeing patient care from the time of admission until discharge, ensuring seamless transitions and optimal clinical results. They serve as a patient and family advocate, working closely with a multidisciplinary team to effectively manage the length of stay and resource allocation. Applicants should possess a Master’s degree in Social Work from an accredited institution. Familiarity with software and electronic medical record (EMR) systems is essential, and relevant licensure or certification may be preferred based on the specific level of the position.
