**Job Title: Transition Facilitator**
The Transition Facilitator is tasked with delivering case management, offering life skills education, and providing job support to young adults participating in a transitional housing initiative. This role involves managing intake processes, executing crisis intervention strategies, and ensuring accurate documentation, all while working in partnership with local support services. Ideal candidates will possess a minimum of one year of experience in supporting adolescents or young adults within residential or homelessness-related programs. A bachelor’s degree is highly desirable, and applicants must have a valid driver’s license and reliable vehicle for community engagement activities.
