**Transitions Care Coach**
The Transitions Care Coach delivers personalized education and assistance to patients within their residences, focusing on the management of chronic illnesses and the reduction of hospital readmissions. This role involves working closely with patients to set health objectives, access necessary resources, and encourage self-management techniques. Applicants should have graduated from an accredited nursing program and possess an active, unrestricted RN license. A background of 1-2 years in home health or care coordination is desirable, alongside excellent communication and computer proficiency.
