Job Summary
The role involves collaborating with a diverse healthcare team to perform precise patient assessments and develop or update personalized care plans. The position requires effective communication with patients and their families to foster involvement in care decisions while overseeing the work of assigned LPNs and nursing partners. Additional responsibilities may be assigned by the Director of Nursing.
Responsibilities
- Coordinate with the interdisciplinary care team to ensure accurate assessments of patients.
- Develop or modify individualized care plans as necessary.
- Engage with patients and their families to promote active participation in care choices.
- Supervise the work of LPNs and nursing partners.
- Perform any other tasks as instructed by the Director of Nursing.
Qualifications
- Graduation from an accredited nursing program.
- A current RN or LPN license in Alabama.
- Preferred two years of nursing experience working with geriatric patients.
- Demonstrated leadership abilities.
