**Licensed Practical Nurse (LPN)**
The LPN plays a key role in providing high-quality, outcome-driven nursing care while working closely with the interdisciplinary team to create and execute personalized care strategies. In addition, they support discharge planning and ensure consistent communication with physicians and other healthcare team members. Applicants are required to have a valid and current Licensed Practical Nurse (LPN) license in good standing, along with a current CPR certification. This position demands a compassionate and collaborative team member who is available to work three 12-hour shifts during the weekends.
