Job Summary
CentraCare is looking for a dedicated Nurse Practitioner or Physician Assistant to become a vital member of our Weight Management team in St. Cloud, MN. This position calls for a professional eager to contribute to our mission by working alongside physicians, providers, and registered dietitians to deliver exceptional care to patients struggling with weight management.
Responsibilities
- Collaborate with an interdisciplinary team to design and implement weight management plans for patients.
- Conduct comprehensive assessments of patient health to identify weight-related issues and establish personalized treatment strategies.
- Educate patients regarding weight loss strategies, nutritional guidelines, and lifestyle modifications.
- Monitor patient progress and adjust care plans as necessary to promote optimal outcomes.
- Participate in community outreach and educational programs related to weight management and health promotion.
Qualifications
- Active Nurse Practitioner or Physician Assistant license in Minnesota.
- Experience in weight management, nutrition, or a related field is preferred.
- Strong communication and interpersonal skills to effectively work with diverse patient populations.
- Commitment to continuous learning and staying updated with the latest in weight management practices.
- Ability to work collaboratively within a multidisciplinary team environment.
