Job description
The Home Health Physical Therapist is responsible for delivering therapeutic services in patients’ homes. This role includes evaluating patient mobility and strength while formulating personalized care plans. Additionally, the therapist will oversee the work of physical therapy assistants and collaborate with families to facilitate optimal patient recovery. Applicants must possess at least a bachelor’s degree in physical therapy and hold a current state license to practice. A minimum of two years of professional experience is essential, with priority given to candidates with experience in home health or community environments.
