We are in search of a committed Nurse Practitioner or Physician Assistant to become a vital part of our expanding Post-Acute Care team in North Conway, New Hampshire. The role is focused on delivering exceptional patient care through collaboration within interdisciplinary teams, aiming to effectively manage both chronic and acute medical conditions.
Responsibilities
- Collaborate with multidisciplinary teams to provide comprehensive care.
- Manage treatment plans for patients with ongoing chronic and acute medical conditions.
- Support patient recovery while implementing strategies to reduce hospital readmissions.
- Participate in patient assessments and clinical evaluations.
- Educate patients and families about care plans and health management.
Qualifications
- Current certification as a Nurse Practitioner or Physician Assistant.
- Experience in post-acute or similar healthcare settings is preferred.
- Strong collaborative skills and ability to work within a team.
