Responsibilities
- Deliver comprehensive healthcare to geriatric patients.
- Collaborate with healthcare teams in skilled nursing and long-term care facilities.
- Perform patient assessments and develop care plans.
- Administer treatments and medications as prescribed.
- Educate patients and families on health management and disease prevention.
Qualifications
- Certification as a Nurse Practitioner or Physician Assistant.
- Experience working with geriatric patients is highly desirable.
- Strong communication and interpersonal skills.
- The ability to work collaboratively in a team-oriented environment.
- Current state licensure to practice in Colorado.
