Conduct in-home physical therapy sessions, encompassing initial evaluations, subsequent reassessments, and assessments for fall risk. Ensure thorough documentation of patient progress and maintain clear communication with the healthcare team. Applicants must hold an active license as a Physical Therapist or Physical Therapist Assistant in Hawaii. A minimum of one year of clinical experience is required, along with a valid driver’s license and dependable transportation.
