Job description
**Title: Home Health Nurse**
Deliver personalized, one-on-one nursing care to individuals in their residences, which includes the administration of medications and execution of skilled nursing procedures. Continuously assess and monitor patient health, record any changes in their condition, and provide education to patients and their families regarding health management practices. An active RN or LPN license in Illinois and current CPR/BLS certification are mandatory. Prior experience in private duty or home health nursing is advantageous, along with excellent clinical and communication abilities.
