Job description
The Physical Therapist is responsible for conducting thorough evaluations and delivering treatment for PACE participants at various stages, including acute care, restorative care, and maintenance. As a vital part of the Interdisciplinary Team, they play a key role in providing clinical assessments and in shaping comprehensive care plans that address the needs of the individual. Applicants must possess a Bachelor’s Degree in Physical Therapy and hold an active license as a Physical Therapist in California. A minimum of one year of experience in working with elderly or frail populations is essential, along with current CPR/BLS certification.
